TogetherTeam is a transformational care delivery model that leverages the intersection of people, process and technology to inform the dissemination of care processes promoting the highest quality and most effective care in alignment with Trinity Health. The Director, TogetherTeam Change Leadership leads the change management process, ensuring decisions follow proper process through the TogetherTeam’s Executive Steering Committee.

Under the direction of and in partnership with the VP, Transformation Officer, leads a team of Change Leaders dedicated to developing plans and materials to support and coach the Regional Health Ministries, National Health Ministries, System Office Shared Services, and the program team through all change-related aspects and phases of the TogetherTeam implementation, maintaining a common vision, plan, and goal across all areas of the program. Collaborates with System Office leaders, Health Informatics, Trinity Health Services (TIS), and the team of Change Leaders in developing coaching plans and materials for Regional Health Ministry Executives and local clinical leaders to transform and lead change through a clear and visionary care model, integrating the use of Health Information Technology (health IT) throughout Trinity Health and across the continuum of care.

Leads the Change Leadership Team in supporting TogetherTeam as they optimally implement the TogetherTeam care models, ensuring alignment with Trinity Health enterprise goals in advancing care and continuing to develop new care models that leverage the virtual platform in additional care areas and parts of the care continuum. In collaboration with Human Resources, ensures the development of learning and development programs that encourage personnel essential to the TogetherTeam model to be retained and advance in skill development within Trinity Health and TogetherTeam.

MINIMUM QUALIFICATIONS

  1. Bachelor’s degree in Nursing, Business Administration, Health Informatics, Healthcare Administration, or related area, or equivalent combination of education and experience. Master degree preferred. Coaching certification preferred.

  2. Must possess a minimum of seven (7) years leadership experience with and comprehensive knowledge of health care operations and program management at the enterprise level and/or national level. 3. Demonstrated and proven experience in leading transformational change, including knowledge and use of change leadership models and process redesign.

  3. In depth experience in the planning, development, implementation, evaluation, and orchestration of complex clinical systems changes and the incorporation of technologies to improve patient care and the health care delivery process.

  4. Ability to operate in an ambiguous and highly matrix organizational structure. Ability to operate in a highly autonomous self-directed manner under frequently changing structures and requirements and work priorities.

  5. Strong consulting and execution skills. Ability to define program or process actions to achieve department strategy, goals and performance measures.

  6. Exceptional ability to oversee and direct staff through leadership, management and facilitation skills. Demonstrated ability to skillfully counsel, collaborate and negotiate; inspire and build confidence in others; create alliances; obtain support and respect from diverse groups and foster an understanding and commitment to the ministry. Ability to adapt style and approach to match the needs of varying teams, individuals and situations.

  7. Highly developed verbal and written communication, persuasion and negotiation skills in order to effectively identify and present services, assess organizational development opportunities, facilitate improvements, and present advisory, persuasive and authoritative recommendations to all levels of the organization.

  8. Analytical ability to conceptualize well in a fast-paced, dynamic and multi-functional environment requiring creativity, innovation, foresight and mature professional judgment. Ability to operate in a collaborative, shared leadership environment.

  9. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.
